IS INVISALIGN RIGHT FOR ME?
What is Invisalign, and who is it suitable for?
Invisalign is a set of clear, removable plastic aligners, custom-made for your teeth and worn in a planned order so that each one moves the teeth a fraction of a millimetre. It suits most adults and teenagers with mild to moderate crowding, gaps or bite problems, and it depends on you wearing the aligners 20 to 22 hours a day. Severe crowding, teeth that need turning a long way round, and cases where the jaws themselves are different sizes are usually more predictable with fixed braces.
Is Invisalign as effective as fixed braces?
For mild to moderate crowding and spacing, studies that score finished results find no meaningful difference between Invisalign and fixed braces. Fixed braces still control tooth rotation and root angle more reliably, so they remain the better option for badly rotated teeth, large bite changes, and cases where teeth have to be removed. More than half of aligner cases need extra refinement aligners at the end, or a short spell with fixed braces, to finish the fine detail. That is normal and planned for rather than a sign that something has gone wrong.
Can Invisalign fix my bite as well as straighten my teeth?
Invisalign can correct many bite problems, including mild to moderate overbites, deep bites, crossbites and small open bites, usually with the help of bonded attachments and small elastic bands hooked between the upper and lower aligners. Bite correction is slower and less predictable than simple straightening, because teeth have to be tipped and lifted rather than just lined up. Where the bite is caused by the upper and lower jaws being different sizes, fixed braces, and occasionally jaw surgery, may be needed instead of aligners alone.
My teeth have moved since I had braces as a teenager. Can Invisalign straighten them again?
Yes, and relapse after childhood braces is one of the most common reasons adults ask about Invisalign. Treatment is often shorter than the original course, because the teeth have usually drifted only a few millimetres, most often the lower front ones. Retainer wear stopping is nearly always the cause, so the same drift will happen again unless you wear a retainer at night long term afterwards. The dentist will also check that nothing else has changed, such as gum recession or a tooth that has since been crowned.
Can teenagers and children have Invisalign?
Yes. Teenagers are usually treated once most adult teeth have come through, often from around ages 12 to 13, and aligners for this age group include small blue dots that fade with wear so parents and the dentist can see how much they are actually being worn. Younger children with a mix of baby and adult teeth can sometimes be treated with a version designed to guide jaw growth and make room for adult teeth. Suitability depends on how far the teeth have developed and whether your child will wear the aligners, not on age alone.
Can I have Invisalign if I have crowns, veneers, a bridge or an implant?
Usually yes. A tooth with a crown or a veneer still has its own root, so it moves normally with aligners. An implant is fixed into the bone and cannot be moved at all, so the plan is designed around it. A bridge joins teeth together, which limits how far that section can move, and it sometimes needs replacing afterwards. Attachments also grip porcelain less well than natural enamel, so they may be placed on neighbouring teeth instead. If you are considering new crowns or veneers, straightening first usually means less of the tooth has to be cut away.
Do my teeth and gums need to be healthy before I start Invisalign?
Yes. Active decay and gum disease have to be treated and stable before aligners start, because moving teeth through inflamed gums or infected bone can loosen them and cause further bone loss. Expect an examination, X-rays and usually a hygienist appointment first; a hygienist visit is from £80 and X-rays are £15. If you have had gum disease in the past, treatment can still go ahead once it is under control, although small triangular gaps may show near the gum line as teeth straighten, because the bone and gum there have already shrunk back.
Are there any risks with Invisalign?
The main risks with Invisalign are tooth decay and gum inflammation if teeth are not cleaned before the aligners go back in, some gum recession, and a small amount of shortening of the tooth roots, which happens with all orthodontic treatment and rarely causes trouble. Teeth often feel sensitive or slightly loose while they are moving, and that settles. No result can be guaranteed: the finished position depends heavily on your wear time, and some cases need extra aligners, or a change to fixed braces, to finish properly.
Are cheap clear aligners posted to my home the same as Invisalign?
No. Aligners ordered online and made from an impression you take yourself are not the same as treatment planned and monitored in a dental practice. The General Dental Council states that there is no effective substitute for a physical clinical examination before orthodontic treatment, and that you must know the name of the dentist responsible for your care and be able to contact them directly. Without an examination and X-rays, decay, gum disease and root problems get missed, and moving teeth in an unhealthy mouth can loosen them.
Who will carry out my Invisalign treatment, a dentist or an orthodontist?
Any dentist registered with the General Dental Council can provide Invisalign once trained in it, and many general dentists treat straightforward cases routinely. A specialist orthodontist has completed at least three further years of training and appears on the GDC specialist list, which counts for most with complex bite problems, cases needing teeth removed, and children whose jaws are still growing. Ask who will plan your case, how many they have treated, and whether harder cases are referred on. You are entitled to know the name of the dentist responsible for your treatment.
Is Invisalign available on the NHS?
No. NHS orthodontic treatment is normally fixed metal braces, and it is only free for under-18s whose teeth are affecting daily life, for example by making eating or cleaning difficult. Adults almost always have to pay for braces privately. Clear aligners are not routinely provided on the NHS at any age, so Invisalign at Oakgrove Dental Practice is a private treatment, from £2,800.
COST, TIME & GETTING STARTED
How much does Invisalign cost?
Invisalign at Oakgrove Dental Practice is from £2,800, and the new patient cosmetic consultation that comes first is £130. What you pay depends on how far the teeth have to move and how many aligners that takes, so you are given a written quote after the consultation and before any treatment begins. Retainers to hold the result, and any fillings or hygiene treatment needed before you start, are charged separately. Prices may vary depending on location.
What happens at an Invisalign consultation?
An Invisalign consultation covers an examination of your teeth, gums and bite, X-rays, and a digital scan of your mouth that replaces the older putty impressions. The scan is used to build a three-dimensional preview showing how your teeth would move, how many aligners it would take and roughly how long, so you can see the likely result before committing to anything. A new patient cosmetic consultation is £130, X-rays are £15, and a CT scan, if one is needed, is £150.
How long does Invisalign treatment take?
Most Invisalign treatment takes 6 to 18 months. Small corrections to the front teeth sit at the shorter end, while cases involving bite changes or extractions can run closer to two years. You are given a predicted number of aligners at the start, each worn for one to two weeks, and that is what sets the timescale. Treatment runs longer than predicted if aligners are worn for fewer than about 20 hours a day, or if extra refinement aligners are needed at the end.
Will I need teeth filed down or teeth taken out to make space?
Many Invisalign plans include filing a tiny amount of enamel from between the teeth to make room, which is called interproximal reduction, or IPR. Usually 0.1mm to 0.5mm is taken from each contact point, out of enamel roughly 2.5mm thick, using a fine strip or disc. Enamel contains no nerves, so IPR does not normally need an injection and should not hurt. Having teeth out is much less common with aligners than with fixed braces, and where crowding is severe enough to need an extraction, fixed braces are often the more predictable choice. Extractions by a general dentist are £165 to £250 depending on the tooth.
DURING TREATMENT
How does Invisalign treatment work, and how often will I need check-ups?
Invisalign starts with a digital scan of your mouth, which is used to plan every tooth movement and make the whole series of aligners in advance. You wear each aligner for one to two weeks before moving to the next, and review appointments are usually every 6 to 8 weeks to check the teeth are tracking to plan and to hand over the next batch. Small attachments are bonded to some teeth at the first fitting, and any enamel filing is done a little at a time as treatment progresses.
How many hours a day do I need to wear the aligners, and is it painful?
Invisalign aligners need to be worn 20 to 22 hours a day, coming out only to eat, to drink anything other than water, and to brush. That leaves two to four hours a day, which three meals and brushing will easily use up. Each new aligner feels tight and makes the teeth ache for two to three days, more pressure than pain, and it eases as the teeth move. Ordinary painkillers help, and biting on the small foam cylinders called chewies seats the aligner fully, which often reduces the ache.
What happens if I do not wear my aligners for long enough?
If Invisalign aligners are worn less than about 20 hours a day, the teeth fall behind the plan, the next aligner will not seat fully and treatment takes longer. Teeth begin drifting back within a day or two of the aligners being out, so missed hours cannot be made up later by wearing them longer. Where the fit has been lost, the usual fix is to return to the last aligner that fitted properly, wear it full time, then rescan and order extra aligners. Being honest about your real wear time at reviews makes that far easier to correct.
Can I eat, drink and clean normally with Invisalign in?
Invisalign aligners come out to eat and to drink anything except plain water, and teeth should be brushed, or at least rinsed, before they go back in. Trapping sugar or acid against the teeth under an aligner raises the risk of decay considerably. Hot drinks warp the plastic, and tea, coffee, red wine and cola stain it. Clean aligners with cool water and a soft brush rather than toothpaste, which is abrasive enough to scratch them cloudy. Always use the case: aligners wrapped in a napkin get thrown away.
What are attachments or buttons used with Invisalign?
Attachments are small tooth-coloured bumps of filling material bonded to certain teeth so the aligner has something to push and pull against. They are what allow an aligner to turn a tooth or move its root, rather than simply tipping the crown. Most people have several, commonly on the canines and back teeth. They feel rough to the tongue for a few days and are hard to spot. At the end of treatment they are polished off without drilling into the tooth. Separate small hooks are sometimes added to hold elastic bands.
Will Invisalign affect my speech?
Most people have a slight lisp for the first few days with Invisalign, because the aligner covers the back of the upper front teeth, which is where the tongue makes s and z sounds. It usually settles within a week as the tongue adapts, and it often comes back briefly with each new aligner for the first few sets. Reading aloud for a few minutes a day speeds that up. If a lisp is still obvious after a fortnight, mention it at your review, as an aligner edge can be trimmed.
What happens if I lose an aligner?
If you lose an Invisalign aligner, put your previous one back in straight away and wear it full time, then phone the practice, because teeth left with no aligner at all start to drift. This is why every old aligner is worth keeping. Depending on how far through that stage you were, you may be told to move to the next aligner early, or a replacement may be ordered, which usually takes a couple of weeks. Never skip several stages to catch up: the aligner will not seat and the teeth get overloaded.
Can I smoke or vape while wearing Invisalign?
Invisalign aligners should come out before smoking or vaping, and teeth brushed before they go back in. Tar, nicotine and the dyes in e-liquids stain the plastic yellow or brown within days, which defeats the point of a clear aligner, and heat from smoke or vapour can distort the thin plastic so it no longer fits. Nicotine also narrows the blood vessels supplying the gums, which slows healing and makes gum problems more likely while teeth are moving. Treatment is a good moment to stop, and your dentist can point you towards NHS stop smoking support.
AFTER TREATMENT
Do I need retainers after Invisalign, and for how long?
Yes. Retainers are needed after Invisalign for as long as you want the teeth to stay straight, which in practice means wearing them at night indefinitely. Teeth tend to drift back towards their old positions, and they also keep shifting slowly with age whether or not you have had braces. You may be given a clear removable retainer that looks like an aligner, a thin wire bonded behind the front teeth, or both. Clear retainers wear out and usually need replacing every one to two years. Retainers are charged separately from the Invisalign fee, so ask for that figure in your written quote.
What happens if I stop wearing my retainers or lose one?
Teeth start to move within days of retainers being left out, and noticeable crowding of the lower front teeth can return inside a year. If a retainer is lost or broken, contact the practice within a few days: while the teeth are still in position, a replacement can usually be made from a fresh scan. After months without one, the teeth may have drifted far enough that a short course of aligners is needed before a new retainer will fit. A bonded wire that feels loose needs checking quickly, because a partly detached wire can move teeth the wrong way.
Can I whiten my teeth as well as straightening them?
Yes, and most people whiten after Invisalign rather than during it, because whitening gel cannot reach the enamel underneath the bonded attachments, which leaves paler patches once they are polished off.
